Death Registration (SHIBO-TODOKE)
- Period: Within 7 days from the date of death
- Place: At an appropriate administrative office in the area where the deceased person died or was living, or the applier is living.
- In Chino City:
If you wish to cremate the deceased person, please make an appointment at Resident Registration Division by phone.
Cremation permit is required. Please submit the death registration and receive the cremation permit.
When the deceased person is registered at Chino, Suwa, Fujimi, or Hara, the cremation fee at SEIKA-EN is 5,000yen. When the deceased person is registered at other places, please make an inquiry at Civic Affairs Division (Shimin-ka) #2 (ext. 252) for details.

Other Proceedings related to Death Registration
- If the deceased person was covered by National Health Insurance Plan: Health Division (Hoken-ka) #8 (ext. 323)
- If the deceased person was covered by other health insurance plans, please make an inquiry to an applicable office.
- To return the recipient certificate for Medical Care for the Elderly: Health Division (Hoken-ka) #8 (ext. 323)
- To return the certificate of Nursing Care Insurance: Health Division (Hoken-ka) #9 (ext. 334)
- If the deceased person was covered by National Pension Plan or was a recipient of National Pension: Civic Affairs Division (Shimin-ka) #2 (ext. 252)
- If the deceased person was a recipient of Disability Pension Plan, Bereaved Family Pension Plan, or Widow’s Pension Plan: Civic Affairs Division (Shimin-ka) #2 (ext. 252)
- If the deceased person was a recipient of pension payment from National Pension Plan or Employee’s Pension Plan, or was covered by Employee’s Pension Plan, please make an inquiry to Social Insurance Office in Okaya for details.
- If the deceased person was covered by or a recipient of mutual aid pensions, please make an inquiry to applicable mutual aid associations.
- Please report the death to an appropriate administrative office of your country. Make an inquiry to the embassy or consulate of your country for details.
National Health Insurance Plan will provide 50,000yen of Lump-sum Funeral Allowance to a host of the funeral. Please inform Insurance Division of the bank account of the host.
