Birth Registration (SHUSSYO-TODOKE)(for a child who was born in Japan)

  • Period : Within 14 days from the date of birth
  • Applier: The child’s father or mother
  • Place : At the city/town/village hall where the mother or father is living
  • (if the couple are not married, where mother is living) or where the child was born.)

Birth Registration Procedures at Chino City Hall

  • Please submit your child’s birth registration at Civic Affaires Division (Shimin-ka) #2 (ext. 252)
  • Documents needed for birth registration
  • Birth Registration Form (Available at city/town/village halls or hospitals)

    Birth Certificate with verification by a doctor or midwife who served for the birth

Other Proceedings related to Birth Registration at Chino City Hall

  • Posting service for newspapers and TV is free of charge.
  • If you wish, your child’s birth will be posted on Shimin Shinbun and Nagano Nippo as well as LCV’s tele-text broad casting.

  • Application for Child Allowance Program at Civic Affaires Division (Shimin-ka) #4 (ext. 256)
  • Child Allowance is provided for a family member who is raising a child who is younger than the grade 4 in elementary school and whose income is less than a certain amount. To receive the allowance, you need to apply for the program.

    Please apply for the program within 15 days after your Child’s birth.

    5,000-10,000 yen per month depending on the number of children you are raising.

  • Guidance about Child Medication System; Health Check-up; Immunization Schedule at Health Division (Hoken-ka) #8 (ext. #323)
  • Child Medication System supports a part of medical costs for your child before school age.

    Health check-up is available when your child is 4 months, 10 months, 18 months, and 3 years old.

    Dental check up is available when your child is 2 years old.

    Immunization notebook will be handed to you.

  • Application for Lump-sum Birth Allowance at Health Division (Hoken-ka) #8 (ext. 323)
  • The allowance will be provided to the ones who are covered by National Health Insurance.

    For ones who are covered by other health insurance, please make an inquiry to your company (If your child was born within 6 months after your retirement, the allowance may be provided from the company. In this case you don’t receive the allowance from National Health Insurance).

    Allowance: 350,000 yen.


  • If your child was born abroad, please contact Resident Registration Division.
  • Please report your child’s birth to an appropriate administrative office of your country.
  • Please apply for the child’s resident status to Immigration Office within 30 days from the birth.
